Je soussigné/e agissant en mon nom personnel ou en ma qualité de parent ou tuteur de la personne mineure détaillée ci-dessus, déclare par les présentes avoir pris connaissances des règlements et de toutes les conditions relatives à l’initiation de Roller Skating par Nicolas Constantin représentant légal de NICOSPORTS LTD et m’engage tant en mon nom personnel que celui de la personne mineure susmentionnée pour qui je me porte garant/e de me conformer aux dits règlements et de respecter toutes les dites conditions qu’ils furent communiqués en écrit ou verbalement.
Nonobstant la généralité des dispositions continues au paragraphe qui précède, je soussigné/e agissant en mon nom personnel ou en ma qualité de parent ou tuteur de la personne mineure susmentionnée, déclare par les présentes m’engager solennellement tant en mon nom personnel que celui de la personne mineure susmentionnée pour qui je me porte garant/e de me conformer plus précisément aux conditions suivantes.
Je m’acquitterai, dans les délais prescrits par Nicolas Constantin représentant légal de NICOSPORTS LTD, de toute charge relative à l’initiation de Roller.
Je m’engage aussi à m’assurer du port de tout équipement, qu’il soit de protection ou autre, que ladite société indiquera et jugera nécessaire pour l’initiation de Roller Skating.
En cas d’accidents occasionnant blessure ou pas, je donne par les présentes l’autorisation aux préposés de prendre toutes les dispositions qu’ils jugeront nécessaires dans la circonstance et m’engage, de plus, à ne point les tenir responsables, tant au civil qu’au pénal, pour toute disposition prise par eux en vertu des disponibles du présent paragraphe.
Finalement, je déclare et reconnais qu’en aucun cas la responsabilité Nicolas Constantin représentant légal de NICOSPORTS LTD et de ses collaborateurs, employés, représentants, agents, ayant droit et préposés ne pourra être recherchée pour tout dommage ou préjudices ou bien raison de ma participation à l’initiation de Roller Skating par moi ou la personne mineure susmentionnée.
Il est aussi expressément convenu que l’activité « l’Ecole de Patinage » est exclusivement géré par Nicolas Constantin représentant légal de NICOSPORTS LTD et que cette dernière n’est pas en partenariat avec le propriétaire des lieux où se situe l’opération ; le dit propriétaire étant totalement étranger à la convention qui m’unit à Nicolas Constantin par les présentes.
J’autorise mon enfant à faire des photos soit en groupe ou individuellement mais directement lié à la présente animation (presse, télévision, web), toutefois pas à des fins publicitaires.

I, the undersigned, acting on my own behalf or in my capacity as parent or guardian of the minor detailed above, hereby declare that I have read the regulations and all the conditions relating to the initiation of Roller Skating by Nicolas Constantin, legal representative of NICOSPORTS LTD, and undertake both on my own behalf and on behalf of the minor for whom I vouch to comply with the said regulations and to comply with all the said conditions whether they were communicated in writing or orally.
Notwithstanding the generality of the provisions continuing in the preceding paragraph, I, the undersigned, acting in my personal name or in my capacity as a parent or guardian of the above mentioned minor, hereby solemnly undertake on my own behalf and on behalf of the above-mentioned minor for whom I vouch to comply more precisely with the following conditions.

Finally, I declare and acknowledge that under no circumstances can Nicolas Constantin legal representative of NICOSPORTS LTD and its collaborators ,employees, representatives, agents, beneficiaries and attendants be sought for any damage or prejudice or reason for my participation in the initiation of Roller Skating by me or the aforementioned minor.
It is also expressly agreed that the activity "l'Ecole de Patinage" is exclusively managed by Nicolas Constantin, legal representative of NICOSPORTS LTD, and that the latter is not in partnership with the owner of the premises where the operation is located; the said owner being totally alien to the agreement that unites me with Nicolas Constantin hereby.
I allow my child to make photos either in a group or individually but directly related to the present animation (press, television, web), however not for advertising purposes.
